Your choices, your community, your team around you
Personalised, Person-Centred Care
We offer twenty-four-hour support, including waking night provision, to meet daily living, social, emotional and educational needs. Every resident is allocated a keyworker, responsible for coordinating their individual Support Plan, and we work to understand each person’s own means of communication so their views are properly heard and recorded. Our care teams help people build the skills and confidence to take part more fully in everyday life, stay connected to family and friends, and get involved in local community activities — including pubs, social clubs, swimming, church, bowling, and cycling. Staff bring a wide range of backgrounds, skills and experience, beginning with a structured induction and continuing through ongoing NVQ training, regular supervision, and annual appraisals, so every resident is supported by a genuinely skilled and consistent team.
A considered move, and continued peace of mind
Admissions & Quality Assurance
Places at Hylton House are for adults aged 16 and over with physical and/or learning disabilities and additional complex needs. Enquiries go through our Registered Manager, with visits to and from the home built into the assessment process, and a full review involving the individual, their relatives or carers, and an advocate where appropriate — ensuring any move is a genuinely informed choice. Ongoing quality is underpinned by a written Abuse Prevention Policy aligned to local authority procedures, a clear Complaints and Concerns process shared with everyone using the service, and staff trained to identify and report any concerns, so families can be confident support stays safe and accountable long after admission.
